Part Three: Beyond Basics of Case Management Five-Part Series (ODW5012PT3)

This is a recording of a live webinar delivered on April 24, 2025. 

Having a foundation for effective case management is critical. This five-part series will cover solutions for many of the challenges faced by case managers today. 
 

The series will first discuss key stakeholders and their integral role in the rounding process. Next, attendees will learn how to incorporate case management at all access points to your hospital or health system. This will be followed by the often-challenging subject of how best to collaborate with physicians, especially high-volume ones such as hospitalists and emergency department physicians. The series will then review how to remain forward looking in your practice. Finally, the series will discuss how to measure and how to report case management outcomes.

Speaker

Toni G. Cesta, Ph.D., RN, FAAN is a founding partner of Case Management Concepts, LLC, a consulting company that assists institutions in designing, implementing, and evaluating case management departments and models, educational programs, and onsite support for leadership and staff. 

She has been active in the research and development of case management for over 25 years. Her research has included two funded studies measuring the effects of a case management model on congestive heart failure and fractured hip patient populations, with measures of patient satisfaction, quality of life, and short- and long-term clinical perceptions and outcomes.

  • Review case management specialty roles held by RNs and/or social workers.   
  • Develop best practice strategies with key physician stakeholders to ensure optimal outcomes.

  • Identify opportunities for improvement at key locations throughout the hospital while collaborating with physicians and other case management team members.

Attendees who wish to have the hours applied toward ACHE Qualifying Education credit must self-report their participation by logging into their MyACHE account and selecting ACHE Qualifying Education Credit.
