This five-part boot camp provides new case managers with the fundamentals for best practices in the acute care setting.  If you are an experienced case manager, this boot camp will provide a refresher on the fundamentals, as well as updates on regulatory and compliance requirements.  If you are considering becoming a case manager, join this series of webinars to gain knowledge about what it means to be a case manager.

No matter what your level of experience may be, understanding best practices for case managers in a hospital is as important as ever.  Because things are changing so rapidly, it can be a challenge to stay current and knowledgeable on the issues that most impact your role as a case manager. Reimbursement has changed dramatically, as has utilization management, transitional planning, and length of stay management.

This boot camp, designed for both registered nurses and social work case managers, will provide the core knowledge needed to be successful. Each week’s topic will build on the prior one as we move through the roles and functions that are foundational for hospital case managers. The series will conclude with a discussion on how to incorporate all the roles and functions into a best practice framework for you and your department.

At the conclusion of this series, participants should be able to:

Part Three:

  • Understand discharge planning as a process, not an outcome.
  • Discuss the new CMS changes related to transitional and discharge planning and how they can impact your practice.
  • Identify best practice strategies for transitioning patients across the continuum of care.
