Discharge planning is a process that starts at the point of admission and follows beyond discharge through the continuum of care. This workshop will cover a wide range of topics related to discharge planning, including how to engage with other members of the interdisciplinary care team when planning for the patient’s movement across the continuum. Learn how to ensure that your discharge planning processes address the complexities of the new healthcare environment as well as the Centers for Medicare & Medicaid Services (CMS) regulatory requirements.

This webinar series is designed for registered nurses and social work case managers. This series is useful, both for those who are new to the process and those who want to upgrade their role in discharge planning.
